Who this page is for

If your hip is making the decisions, this page is for you.

This page is written for people who have been told they may need a hip replacement, or who suspect they may. You may have been waiting a long time, you may not have been seen yet, or you may have been managing for years and want a straight answer about what comes next.

A private consultation is not a commitment to surgery. It is a clear, focused conversation about what your hip is doing, what your options are, and what the right next step is, including doing nothing for now if that is the honest answer.

1-minute assessment

Is hip replacement right for me?

Nine quick questions and a tailored next step from Professor Lee's team. Not a diagnosis — every result is an invitation to talk it through.

1-minute assessment

Is hip replacement right for me?

Nine quick questions about how your hip is behaving — pain, function, sleep, imaging and what you've already tried. We use it to suggest the right next step: a free discovery call, a private consultation, or a clear conversation about hip replacement with the team led by Professor Paul Lee.

  • We read your hip through four lenses: severity, structure, treatment trail & readiness
  • It is not a diagnosis — the result is always an invitation to talk
  • Suitability for the SPAIRE muscle-sparing technique is confirmed at consultation

This questionnaire is for shared decision-making only. It does not provide a diagnosis or replace specialist consultation, clinical examination or imaging review.

When hip replacement may be considered

Hip replacement is usually considered when these things stack up.

  • Pain is persistent

Pain in the groin, side of the hip or front of the thigh, that does not settle with rest.

  • Sleep is interrupted

You are being woken at night or struggle to find a comfortable sleeping position.

  • Walking distance is shrinking

Your comfortable walking range has dropped, or you find yourself limping on longer walks.

  • Non-surgical measures have plateaued

Physiotherapy, weight management, simple pain relief and selected injections are not giving lasting relief.

  • Imaging confirms structural change

X-ray or MRI shows changes consistent with hip arthritis affecting the joint.

  • It is changing how you live

Sleep, mood, work or activities you used to enjoy are being limited by your hip.

Not everyone with hip arthritis needs surgery. Suitability is confirmed during your consultation, taking your symptoms, imaging and personal goals into account.

What happens during a private consultation

A focused appointment with the hip team led by Professor Lee.

Professor Paul Lee sets the assessment standard the team works to. You will leave the appointment with a clear understanding of what is happening with your hip, what your options are, and what a sensible next step looks like.

  1. 1

    Listen

A short conversation about what you have noticed in your hip, what you have tried and what matters most to you.

  2. 2

    Examine

A focused clinical examination, including range of motion, gait and a check for other potential pain generators.

  3. 3

    Review imaging

Existing X-rays or MRI are reviewed in front of you. If imaging is missing or out of date, it can be arranged.

  4. 4

    Discuss options

Non-surgical and surgical options are talked through honestly, including timings, expected outcomes and risks.

  5. 5

    Plan next steps

If hip replacement is sensible, the team will explain whether the SPAIRE technique is suitable for you and what the package includes.

A note on the method we are best known for

SPAIRE is one technique among several, but it is the one we are built around.

SPAIRE aims to preserve the short external rotators and piriformis tendon during hip replacement, where it can be done safely. It is designed to support early mobility and reduce some post-operative precautions in suitable patients. It is not used as a marketing badge. If another technique is the right answer for your hip, Professor Lee will tell you and explain why.

  • Designed to preserve key stabilising muscles
  • May support early walking for suitable patients
  • Suitability assessed at consultation
  • Hip team led by Professor Lee

FAQs

Frequently asked questions

Do I need a GP referral?
No GP referral is required to book a private consultation. If you would like your GP to be involved we can write to them after your appointment with your consent.
Will I need imaging?
Most patients benefit from up-to-date hip X-rays, and in some cases an MRI scan. If you already have recent imaging you can bring it with you; if not, it can be arranged.
How do I know if I need a hip replacement?
Hip replacement is typically considered when hip arthritis is affecting daily life (pain on walking, stiffness, disturbed sleep) and other measures such as physiotherapy, weight management or injections have not given lasting relief. A private consultation, imaging and examination help confirm whether surgery is a sensible next step or whether other options are worth trying first.
What happens at the first appointment?
A senior member of the hip team reviews your history, examines your hip and goes through any imaging you bring with you. If further imaging is needed it can be arranged. You will leave with a clear understanding of the likely diagnosis, treatment options and whether the SPAIRE technique may be suitable for you. The plan is set under the clinical standard Professor Lee leads.
What is the SPAIRE method?
SPAIRE is a muscle-sparing approach to hip replacement that aims to preserve the short external rotators and the piriformis. It is used where clinically appropriate and is decided on a case-by-case basis after examination and imaging.
Is SPAIRE suitable for everyone?
No. SPAIRE is one approach among several. Suitability depends on your anatomy, the severity of your arthritis, your prior surgeries and your goals. If SPAIRE is not the right fit, Professor Lee will explain why and recommend the most appropriate technique.
Where would surgery take place?
Surgery is carried out at a private hospital in the Harley Street, London. The specific provider is confirmed at the time of booking.
How long does recovery take?
Recovery varies by patient, technique and how active you were before surgery. Most patients are mobilising the same day, return to gentle daily activities within a few weeks, and continue regaining strength over the following months. Your individual recovery plan is set up before surgery.
What are the risks of hip replacement?
All surgery carries risk. The most relevant risks to discuss include infection, blood clots, dislocation, leg-length difference, fracture, and the rare need for revision. These are talked through carefully during your consultation so you can make an informed decision.
